If you are enrolling from a school not in Gaston County Schools,

the following is required in order to complete a regular enrollment at John Chavis Middle School:

 

  • Parent or legal guardian must be present with proof of guardianship and identification.
  • Two (2) Proofs of Residence: These are current documents received within a month (30 days) that include the parents’ name and street address. Examples include: utility bill, rent/lease agreement, homeowner’s verification.
  • Completed Withdrawal Form from Previous School: You need to complete this form at the previous school when you withdrawal your student. This form will include the name, telephone number and complete mailing address of the school from which you are transferring, previous grades, test scores, attendance, student’s current school enrollment status, etc.
  • Student’s birth certificate: We can make a copy here at school.
  • Student’s Immunization records: We must have a copy of the original shot records with a doctor/nurse signature.
  • Completed School Enrollment Packet: You can complete this packet here at our school. We have a notary here on staff to assist you with the form that needs to be notarized. The Enrollment Packet is available from the school registrar and includes:

       * Student Enrollment Form

       * Discipline Status Form

       * Felony Charges and/or Convictions Form

       * Home Language Survey

 

These things are to be completed and given to the school registrar before your student is allowed to be enrolled and attend school at John Chavis.

WITHDRAWAL FROM SCHOOL

 

Students under the age of 18 may be withdrawn from school with parent/guardian permission. Parents/Guardians wishing to withdraw their child must come to the front office to complete necessary paperwork. Upon return of school property, including textbooks, uniforms, etc., and payment of any school fees, procedures will be completed, finalizing withdrawal from school.

This includes:

              ·         Address

              ·         Home phone, cell phone or work phone

              ·         Emergency contact names and/or phone numbers

              ·         Custody

              ·         Important medical information
