Dear Parent/Guardian,
I am excited to be able to assist you with your child’s healthcare needs at school.
If your child has asthma, please click here for the necessary documents.
If your child has a severe allergy, please click here for the necessary documents.
For special diet needs such as food allergies: click here
If your child has diabetes, please click here for the necessary documents.
For all other medication needs, please click here for a medication authorization form.
Immunization requirements for elementary school include Dtap, Polio, MMR, HepB, Varicella. These will be required before the 30th day of school along with a NC Health Assessment
Immunization requirements for 12 year olds/7th graders include Tdap and MCV
These will be required before the 30th day
Immunization requirements for 16 year olds/12th graders include MCV
These will be required before the 30th day
