Student Needs Assessment

Student Needs Assessment

Our School Counselor is seeking permission to give students a Needs Assessment so she can better plan her school counseling program. The link and more information is below.

Needs Assessment Consent Form

A part of the American School Counseling National Model involves giving students a Needs Assessment to ensure the supports, programming, and lessons planned for the year are aligned with the needs of the student community. This is a brief survey that asks students about their general educational and wellness needs.

In compliance of Senate Bill 49, we are advertising our needs assessment for you to review.  Parent/Guardian consent must be obtained for your student to participate in the needs assessment.

Please complete the Google Form for consent and to preview the questions that will be asked in the needs assessment. It is linked in this email.

The needs assessment will be given to students whose parent/guardian has given consent during the week of November 27th during weekly lessons with the School Counselor.

Feel free to reach out to our school counselor, Mrs. Griffin, if you have any questions.
